178 Cardiac manifestations of igg4-related disease

Introduction IgG4-related disease (IgG4-RD) is an immune-mediated fibro-inflammatory condition that affects multiple organ systems. Cardiovascular involvement has not been systematically studied, though case reports of putative with severe consequences for affected patients have emerged. We aim to investigate and define the novel cardiovascular phenotype IgG4-RD using magnetic resonance (CMR) imaging. Methods recruited 11 (6 female, 61 ± years, 9 active (8 pancreatic involvement, 3 parotid, 5 bile ducts, kidneys cardiovascular)) histologically confirmed from a tertiary referral clinic. These underwent CMR scan performed at 1.5T including cine images, myocardial tagging, native T1 mapping, late gadolinium enhancement (LGE) extracellular volume (ECV) Reference values were generated 10 healthy controls no cardiac (50% 35 8 years). For T1-mapping, least 15 each sex (15F 17M) used establish sex-specific local normal ranges per guidelines. Results are presented as mean standard deviation unless otherwise stated. Results Patients had similar geometry reference group, average left atrial (LAV) 80 29ml, right (RAV) 72 46ml, ventricular end-diastolic (LVEDV) 144 32ml (RVEDV) 150 45ml (Table 1). Average ejection fractions (LVEF RVEF) 60 4% 59 4%, respectively showed significantly reduced global longitudinal strain (GLS) (-16.8 2.3% vs -18.7 1.6%, p=0.045) Male (n=5) group higher value relative 3/5 male having abnormally high (>2SD above limit normal). Female (n=6) Seven LGE, 6 these in non-ischaemic pattern ECV was statistically different (29.5 2.5% 27.9 value, p=0.192) Conclusion this cohort demonstrated signs subclinical systolic dysfunction, elevated times fibrosis. Future work larger cohorts should seek further phenotypes effects systemic immunosuppressive therapy on involvement. Conflict Interest None
